Refs forgot to add on penalty?

I think it was either in the 3rd or 4th quarter. It's 2nd and 2, chiefs ball. Jason Dunn holds John Lynch and gets a 10 yard penalty. On the next play its 2nd and 2 again. Larry Johnson runs for 3 yards and gets the first down. I'm pretty sure it should have been 2 and 12, but the refs didn't move the ball back. How did that get missed?

I think it was a spot fowl. Consequently it was assessed from the end of the play. But I could be wrong.

they also never tacked on the time back to the clock for the late delay of game penalty, not like a couple seconds woulda changed anything but maybe.

It was a spot foul. But the hold was back at the line of S.

It was a spot holding foul on the run by Tony Richardson, committed by Chris Horn. And it was 2nd and 4, not 2nd and 2 after the play. And yes Larry Johnson picked up the first down on the next play.
