"New Era Dog Ears 59FIFTY 2012 Winter Sideline Hat" is the name of the hat, but it may be hard to find in stock. The site I found it on was sold out of the Broncos version only.

That one says "youth" size. I couldn't find one on there for adults. hmmm...
